精神看護実習において看護学生に生起したinvolvementの概念分析とその多軸評定の作成

Abstract

背景 近年、わが国の看護において、involvementに関する研究が行われ始めている。involvementは、様々な側面に焦点が当てられ看護の中心的な概念であると示唆されている。しかし、看護におけるinvolvementの概念分析は行われておらず、精神科の看護においては、その概念やアセスメントを共有することは難しいのが現状である。したがって、精神看護実習において、患者に対する看護学生のinvolvementを、看護学生や実習指導者、教員が共有するために、その概念分析を行い、測定用具を作成することが必要である。目的 精神看護実習において看護学生に生起したinvolvementの概念を明確にし、その測定用具を作成することを本研究の目的とした。方法 看護学生40名を対象に、ハイブリッド・モデルを用いてデータ収集および分析を行った。結果 精神看護実習において看護学生に生起したinvolvementの作業定義を「対人関係の過程で生起した、経験の共有・感情の投資・絆の形成・境界の調整で構成される看護現象である」とした。また、作業定義で明らかになったinvolvementの4つの構成要素(経験の共有・感情の投資・絆の形成・境界の調整)が、detachment(切り離された)-over-involvement(過剰な)-nursing involvement(適応的な)を軸に、どのように変化するのかを表した「看護学生におけるinvolvement多軸評定」を作成した。結論 これらの結果により、精神看護実習において、教員および臨地実習指導者と学生がinvolvementに関してその概念や程度、注意点などを共有できると考えられる。

Background In Japan, studies on involvement in nursing are gradually becoming more common after the recent reviews of the evaluation of involvement in other countries. But in the past, involvement in nursing was considered to be a difficult issue in Japan and other countries from the employment ethics and objective scientific perspectives. In recent years, various aspects of involvement have been investigated and the importance of this concept has been recognized. It, however, has not been analyzed; therefore, it remains difficult to obtain widespread recognition of the concept of involvement in nursing. Objective The present study aimed to clarify the involvement of nursing students during a psychiatric nursing care practicum, and develop a tool for measuring this involvement. Method The study collected and analyzed data from 40 nursing college students using the Hybrid Model of Schwart-Barcott & Kim. Results From the results of this analysis, a working definition of involvement in the practicum was identified as "the nursing phenomenon that consists of sharing experiences, investing emotions, forming bonds, and negotiating boundaries, and that occurs in the process of human interaction." A "multiaxial assessment for involvement" was developed to describe changes in these four constituents associated with detachment, over-involvement, and nursing involvement. Conclusion Results from this assessment will benefit teachers, practicum instructors, and students by allowing them to share the concept, degree, and important aspects of involvement.
