精神科看護師による急性期にある精神疾患患者の家族への看護

Abstract

背景 精神科急性期病棟では、激しい精神症状を呈する患者や、初発患者が対象となる。こうした患者の場合、その家族もまた様々な不安を抱えている。実際家族が最も緊張が高いと考えられる入院当初では、看護師により実際どのような介入が行われているのかを知ることは、系統的な家族ケアを構築していく上で重要であると考える。 目的 精神科看護師が急性期の患者をもつ家族に実施しているケア内容を明らかにするとともに、急性期における家族への看護介入の構造を検討する。方法 精神科に5年以上勤務する看護師10名に対し、急性期の患者家族に対して、実践していると認識している「ケア実践」等について半構成面接を実施し、質的な分析を試みた。 結果 インタビューの中で語られた家族ケアに関する内容50フレーズをコード化し、類似した内容をカテゴリー化した。家族機能への看護が介入として「行動領域」への介入8項目「認知領域」への介入7項目、「感情領域」への介入12項目、合計27項目が抽出された。結論 精神科病棟に勤務する看護師は、急性期という特異な状況にある患者の家族に対し、「認知」「感情」「行動」の各領域に対して、家族システムの再構築に向けて構造的な介入の実践が行われていた。

Background In acute psychiatric wards, patients with severe psychiatric symptoms or those experiencing the first episode of psychiatric disorders a treated. Families of such patients should also feel anxiety, particularly during the early stages of hospitalization. In order to carry out systematic family care, it is important to clarify the contents of interventions actually being performed by nurses. Objective The present study aimed at both clarifying the type of care that was actually implemented by psychiatric nurses toward families of acute phase patients, and examining the structure of nursing interventions for such families. Methods Subjects were 10 nurses who had worked at a psychiatric department for 5 or more year s. Semi-structured interviews were conducted regarding "care being practiced" which had been offered to families of acute phase patients. Data were qualitatively analyzed. Results Contents of family care obtained from the interviews consisted of 50 phrases, which were codified. Similar content was then categorized. According to the interve nursing interventions directed to family functioning included 8 items for interventions related to the behavior domain, 7 items for related to the knowledge domain, and 12 items related to the emotional domain, culminating in a total of 27 items. Conclusion Psychiatric ward nurses carried out structured interventions aiming at rebuilding the family system in those three domains of knowledge, emotion, and behavior, to families of patients during the specific context of the acute phase.
