精神科看護師による上手くいかなかったという思いのある事例解釈の変化 : 看護における「かかわり(involvement)」を学習して (研究ノート)

Abstract

背景 「かかわり」が重要な位置を占める精神科看護師は、客観的なコミュニケーションスキルなどの対人関係技術を、価値観や感情をもった自分自身を道具とした自分仕様の「かかわり」に仕立てていくことが必要である。看護における「かかわり（involvement）」自体に焦点をあて、系統的に学習することで、精神科看護師が自分自身の「かかわり」に自信を持つことにつながると考えた。 目的 本研究は「かかわり（involvement）」の4視点学習前後の看護師の変化を明らかにすることを目的とする。 方法 精神科病棟に勤務する看護師4名が過去の上手くいかなかったという思いのある患者との「かかわり」の事例解釈の記述を作成する。次に、看護における「かかわり（involvement）」の学習会およびグループディスカッションを行う。その前後にinvolvement関連尺度を用いてinvolvementの傾向を測定する。看護における「かかわり（involvement）」の学習会およびグループディスカッション後に、上手くいかなかったという思いのある患者との「かかわり」の事例解釈を再記述し、involvement関連尺度得点の特徴と合わせて、対象者の変化を分析する。結果 過去の上手くいかなかったという思いのある患者との「かかわり」からも限界設定を行っていた意味など肯定的な側面に気づくことができた。また、気づかずに巻き込まれていたことや、患者との距離の近さや距離の取り過ぎなど、自分自身の「かかわり」に関する傾向に気づくことができた。精神科看護師としての「聴くこと」や「伝えること」など「かかわり」を行っていく上での方向性に気づくきっかけになった。 結論 看護における「involvement (かかわり)」の学習会およびグループディスカッション前後の4名の精神科看護師の変化を、過去の上手くいかなかったという思いのある患者との「かかわり」の事例解釈の記述とinvolvement関連尺度点数を用いて分析した結果、以下の3点が示唆された。1. 過去の上手くいかなかったという思いのある患者との「かかわり」のからも肯定的な側面に気づくことができた。2. 「かかわり」や関係性、距離感などに関する自分自身の傾向に気づくことができた。3. 精神科看護師としての「かかわり」の方向性に気づくきっかけになった。

Background "Involvement" plays an important role in psychiatric nursing. Nurses must use themselves, whatever their values and feelings, to transform objective communication skills and other techniques of interpersonal relations into their own styles of involvement. Our idea was to help psychiatric nurses achieve confidence with involvement by focusing attention on involvement as such so they could learn about it systematically. Objective The study aims to show the changes nurses undergo before and after they learn about involvement from four perspectives. Methods Statements by four nurses working in a psychiatric ward are transcribed as they interpret past cases of involvement with patients that did not seem to go well. This is followed by a study group and discussion of involvement in the context of nursing, before and after which involvement-related scales are used to measure the nurses' involvement tendencies. Changes in participants are analyzed using these statements and characteristics seen in their scores on involvement-related scales. Results Participants became aware that even past involvements with patients that did not seem to go well had certain positive aspects, for instance, in the sense that they had already been establishing boundaries. They also became aware of their own tendencies regarding involvement-for instance, a tendency to become over-involved without realizing it or to grow too close, or keep too great a distance, with their patients. It was a chance for them to become aware of the trajectory of involvement as psychiatric nurses. Conclusion Based on their interpretations of past examples of involvement with patients that did not seem to go well and their scores on involvement-related scales, our analysis of the changes in the four psychiatric nurses before and after group study and discussion of "involvement" in the context of nursmg indicates the following three points: 1. Participants became aware that even past involvements with patients that did not seem to go well had entailed certain positive aspects. 2. Participants became aware of their own tendencies regarding involvement, manner of relating, sense of distance, etc. 3. Participants had a chance to become aware of the trajectory of their involvement as psychiatric nurses.
