精神科看護における看護師の「巻き込まれ」体験の構成要素とその関連要因

Abstract

背景 看護における「巻き込まれ(involvement)」は、否定的側面と肯定的側面から評価されている。特に我が国の精神科看護においては、「巻き込まれ」が問題として扱われることが多い。1980年代以降、「巻き込まれ」に関してはいくつかの研究が行われているが、精神科看護における「巻き込まれ」の体験に関する質的な研究は行われていない。目的 「巻き込まれ」を恐れることなくより有効に自己を利用して患者-看護師関係を深めながら看護する情報を得るため、本研究では、精神科の看護師が患者とかかわる中でどのように「巻き込まれ」を体験しているのか質的に探り明らかにすることを目的とする。方法 関西の公立精神病院に勤務する看護経験年数が5年以上の看護師に面接を行い、得られた逐語録をデータとし、質的帰納的に分析した。結果 関西の公立精神病院に勤務する看護経験年数が5年以上の看護師が体験した「巻き込まれ」には、「看護師の経験不足」と「患者の言動による動揺」による『意図せぬ巻き込まれ』と『主体的巻き込まれ』が含まれていた。『意図せぬ巻き込まれ』から『主体的巻き込まれ』への移行要因は、<看護師の成長>と<チームからのバックアップ>であった。結論 『意図せぬ巻き込まれ』は「巻き込まれ」の否定的側面を、『主体的巻き込まれ』は肯定的側面を反映していると考えられるが、『意図せぬ巻き込まれ』の振り返りはその看護師にとって『主体的巻き込まれ』への移行において貴重なデータとなると考えられる。看護にとって重要な「巻き込まれ」を看護師が自分のものにしていくためには、問題としての「巻き込まれ」とともに看護にとって重要な「巻き込まれ」の理解も深め、成長に必要な「巻き込まれ」の否定的な体験を排除するのではなく、「巻き込まれた」体験を共有し、患者にとってその結果がどうであったのかを振り返ることが重要である。

Background Researchers have examined the positive and negative aspects of nurses' involvement with patients. In Japan, emotional involvement with a psychiatric patient is deemed particularly problematic. Committing oneself to bild a good relationship with a patient is a challenging task. The present study quantitatively explores how nurses involve with psychiatric patients and studies the manner in which a professional therapeutic relationship could be established with a patient. Objective The purpose of the present study is to quantitatively explore how nurses involve with psychiatric patients as well as to identify how they cope with the problem associated with involvement with psychiatric patients. Method I interviewed nurses with five or more years of experience in nursing who were working at public psychiatric hospitals in the Kansai district of Japan. All interviews were audiotaped, transcribed and analyzed both quantitatively and inductively. Results There are two aspects related to nurses' involvement with psychiatric patients: unintended involvement and active involvement. Unintended involvement is often caused by the lack of experience and confusion resulting from the patient's speech and behavior. Active involvement can be achieved through experience and support from other nurses. Conclusion While unintended involvement represents the negative aspect of nurses' involvement with a patient, active involvement represents the positive aspect. Reflecting upon the patterns and consequences of unintended involvement allows nurses to understand what needs to be done to achieve active involvement with a patient without compromising the professional nature of the therapeutic relationship. The results obtained from the present study may help nurses foster a good relationship with patients.
