精神科における男性看護師の役割意識とその関連因子 : 不穏時対応以外を中心に (研究ノート)

Abstract

背景 男性看護師の就業者は大幅な増加傾向にあり、近年は一般科病棟に配属・希望する男性看護師も増加している。男性看護師としての職場も、選択しやすい環境へと変化しつつあるが、精神科では現在でも他科と比較して男性看護師の割合が高い。精神科において男性看護師は、不穏時対応における「力」を求められ、その役割を少なからず感じ、発揮している。しかし、不穏時対応以外でも、男性看護師は女性とは異なる役割を発揮し担っていると考えられる。 目的 精神科における男性看護師の役割意識とその関連因子を明らかにすることを目的とした。とくに、不穏時対応以外の役割に焦点をあてた。方法 近畿圏の精神病院に勤務する男性看護師10名を対象に、男性看護師の役割意識に関する半構成的面接を行った。逐語録を質的帰納的に分析し、順に下位カテゴリーおよびカテゴリーを抽出した。 結果 分析の結果、精神科における男性看護師の不穏時対応を除いた役割意識として、《男性であることを生かした関わり》《男性看護師としての意識はしない》《女性患者への対応で意識をする》の3カテゴリーが抽出された。また、男性看護師の役割意識に関連した因子として、《使命感としての不穏時対応》《男性看護師のイメージをもっている》《少数派であることに関する思い》の3カテゴリーが抽出された。 考察 男性看護師は日常の看護の中で、或いは業務の中で男性であることを自身の強みとして、行動を起こしていることが分かった。これにより、精神科看護において、男性看護師の役割分担に関する示唆が得られた。さらに、少数派である男性看護師の役割が明確になり、女性看護師と協働していく上での一助となると考えられる。 考察 男性看護師は日常の看護の中で、或いは業務の中で男性であることを自身の強みとして、行動を起こしていることが分かった。これにより、精神科看護において、男性看護師の役割分担に関する示唆が得られた。さらに、少数派である男性看護師の役割が明確になり、女性看護師と協働していく上での一助となると考えられる。 結語 1.《男性であることを生かした関わり》《男性看護師としての意識はしない》《女性患者への対応で意識をする》が、不穏時対応を除いた男性看護師の役割意識のカテゴリーとして抽出された。2.《使命感としての不穏時対応》《少数派であることに関する思い》《男性看護師へのイメージを持っている》が、男性看護師の役割意識の関連因子におけるカテゴリーとして抽出された。

Background The number of male nurses is considerably increasing and many of them have sought to work in general medical units in recent years . Their working environments are improving, giving them more latitude in their choice of career. Nevertheless, the ratio of male nurses to female is still higher in psychiatric wards than other wards. In psychiatric wards, male nurses are expected to exert their "physical strength" in a restless situation. They often feel such a role as a duty and actually play this role. Yet, it is probable that male nurses play distinct roles in psychiatric wards even in non-restless situations. Objective The purpose of this research is to clarify the gender role-attitudes and their related factors of male psychiatric nurses in non-restless situations. Methods We conducted semi-structured interviews to 10 male nurses working in psychiatric hospitals in Kinki Region for deciphering their gender role-attitudes. Results We extracted 3 categories and 9 subcategories by analyzing transcripts qualitatively and inductively. Three extracted categories were: 1 ) gender-role awareness in nursing care, 2 ) gender-role unawareness in nursing care, and 3 ) self-conscious care for female patients. As associated factors related to role attitudes of male nurses, three categories were also extracted. They were; 1 ) treating patients in a restless situation due to the sense of vocation, 2 ) having the image of male nurses, and 3 ) self-conscience as a minority. Conclusion This research shows that male nurses have the gender-identity and play roles as men in daily nursing activities and other services. The results provide several insights on how to share roles between male and female nurses, giving some help to manage cooperation between male and female nurses.
